Vaccinations Flu, Shingles, Men ACWY


 Are you in any of the following categories? If so, call now to book into one of the clinics being held from September 2017 if you are unsure we will be able to check for you. 

If you are aged 65 years or over, or suffer from chronic disease (such as heart, lung, liver, kidney or neurological disease), have diabetes, a weak immune system or if you are pregnant then you are entitled to the flu vaccination. Those living in long-stay residential homes or carers should also receive a flu vaccination.


If you are aged 70,71,72,73 or 78,79 please contact the surgery to discuss shingles vaccines.

 Men ACWY Vaccine

All those aged 18 on the 31st August 2017 (born between 1st Sept 1998 and 31st Aug 1999)

Aged 18 on the 31st Aug 2016 (born between 1st Sept 1997 and 31st August 1998)

 Aged 18 on the 31st Aug 2015 (born between 1st Sept 1996 and 31st August 1997)

If you are aged 40 - 74 you are entitled to a free Health Check, It is important to get your health checked, especially if you do not visit the doctors very often. Please call the surgery to book an appointment.


The service will be delivered by West Midlands Ambulance Service NHS Foundation Trust and provides a service to the local population, i.e patients who are registered with GP's in Coventry and Warwickshire areas.

There is an eligibility criteria for using this service.  This may be where the patient's medical conditon impacts on their mobility to an extent whereby they would be unable to acces the care needed, or that the medical condition of the patients is that they require support during their journey.


Patients will be responsible for booking their own transport for outpatient appointment, day procedures and regular treatments such as dialysis, cancer treatment and physiotherapy.  On booking you will need to answer some questions to dertermine eligibility and mobility so we can allocate the correct transport.

Regular users of the service will need to re-book their transport every three months.

  • Name
  • Date of Birth
  • NHS Number
  • Pick up and destination address (including postcode)
  • GP name and practice address
  • Date and time of appointment
